Criminal Background Checks

Policy

The University of Texas Health Science Center at Houston (UTHSCH) is committed to providing a safe environment for its students and employees. Section §51.215 of the Texas Education Code permits UTHSCH to obtain criminal background information regarding applicants for security sensitive positions as designated by the President of UTHSCH or his/her designee. Increasingly, a criminal background check is being required by clinical facilities in which students enrolled in clinical programs receive education and training. Furthermore, some licensing boards in Texas require criminal background checks before issuing a license to practice. Based on this background, The University of Texas Board of Regents at the July 8, 2005, meeting granted authority to all institutions offering clinical programs to amend their catalogs to inform potential students of the possible requirement of a criminal background check. Individuals who are unable to meet the school’s criminal history standards may be denied admission or continued enrollment in the program.

For the purposes of this policy, UTHSCH has determined that all students are in security sensitive positions and thus may be subject to criminal background checks. This policy goes into effect immediately and may encompass current students as well as incoming students. A second background check may be required for clinical placement or other purposes at the discretion of the school and at the expense of the student. (HOOP 6.19)

·        Read through the directions.

·        Click the button for “Yes, I agree to the terms of the statement”

·        Click “Continue”

 Screen 2: Job Search

·        Click on “Student Criminal Background Check Application”

 Screen 3: Criminal Background Check Application

·        Under “To apply for this Job, please enter Your SSN and password” enter your Social Security Number and a password

o       You can make up a password but make sure you will remember it or make a note of it. You may need it again.

·        Click “Apply Now”

 Screen 4:  Qualifying Questions

·        If you have already completed the PayPal transaction and click “Yes”, otherwise go the PayPal, complete payment transaction, and then click “Yes”.

o       PayPal Directions are available on our website.

·        Make sure that you remember your SSN and password from Screen 3. Click “Yes”

·        Enter your date of birth in space provided.

·        Click “Next”

 Screen 5: Basic Information

·        Enter the required information

·        DO NOT CLICK “NEXT”

·        Look on the left side of the screen. Click “Release”

 Screen 6: Release

·        Make sure the information on this screen is correct.

·        Print this screen using the ‘print’ in the first paragraph

·        You will need to sign this release and send it to Pre-Check (address below).

·        DO NOT CLICK “NEXT”

·        Look on the left side of the screen. Click “Final”

It might take you to a screen asking you to confirm the qualifying questions. You can click on the text of “qualifying questions,” confirm them, then click “Final” again.

 Screen 6: Final Confirmation

·        Enter your full name (First and Last Name, no middle initial) in space provided.

·        Enter the last 4 digits of your Social Security Number in space provided.

·        Click “Confirm”
